This dental retractor can also be used as a mouth gag. It has a double ratchet opening mechanism, which locks the gag open and is released by a lever.

 

This retractor cannot be expected to function as secure gags (although, depending on the size and shape of an individual's jaw and teeth, they might be effective in some cases). It is generally possible to push them out of the mouth with a bit of effort. However, by adding some sort of strap around the back of the head to tie them in place, it is possible to make them somewhat more secure.
